By signing this waiver form, I confirm that I have been informed of the potential risks associated with declining wrapping services & package services for my personal items during the move. Washington Utilities and Transportation Commission Tariff 15-C -- Household Goods ITEM 90 - CARRIER LIABILITY FOR HOUSEHOLD GOODS AND CUSTOMER VALUATION OPTIONS 4. (A) Breakage, when items are packed by the customer or the customer’s representative unless it can be proved that the breakage resulted from negligence by the mover in handling the articles. I understand that improper packaging and declining wrapping services may result in damage or loss, for which SONRISE MOVERS LLC cannot be held accountable.
